Position Overview:
We are seeking a dedicated and compassionate Physical Therapist to join our team on a part-time basis. The Physical Therapist will be responsible for providing exceptional rehabilitative care to patients in their homes, helping them recover from injuries, surgeries, or conditions that affect their mobility and function.


Key Responsibilities:

  • Conduct comprehensive assessments of patients' physical condition and functional abilities.
  • Develop personalized treatment plans tailored to individual patients' needs and goals.
  • Implement therapeutic exercises, techniques, and modalities to improve patients' strength, mobility, and range of motion.
  • Educate patients and their caregivers on proper techniques for exercises and activities to promote independence and prevent re-injury.
  • Monitor patients' progress and adjust treatment plans as necessary to achieve optimal outcomes.
  • Collaborate with other members of the healthcare team, including physicians, nurses, and occupational therapists, to ensure coordinated care and maximize patient outcomes.
  • Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans in accordance with regulatory requirements and company policies.

Qualifications:

  • Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.
  • Minimum of 2 years of experience as a Physical Therapist 
  • Current licensure as a Physical Therapist in the state of Virginia.
  • CPR certification.
  • Strong clinical skills and knowledge of evidence-based physical therapy practices.
  • Excellent communication and interpersonal skills, with the ability to effectively collaborate with patients, families, and healthcare team members.
  • Compassionate and patient-centered approach to care.
  • Ability to work independently and manage a diverse caseload effectively.
  • Prior experience in home health care or a similar setting is preferred but not required.

Compensation: $100.00 - $150.00 per hour
