Position Overview:
As an RN Care Manager, you will play a vital role in our organization by conducting client care visits to ensure care needs are being met and adequate care is in place.  You will work closely with caregivers to introduce them to new cases, conduct caregiver supervision visits during shifts to observe, mentor and train caregivers to help them improve their skills. You will be prepared to step in and assist caregivers in case of emergencies.  You will work closely with the office staff to report on client and caregiver issues/concerns and client requests for additional care.


Responsibilities:

  • Conduct weekly client visits to assess care and determine future care needs.
  • Conduct weekly caregiver visits to observe, mentor and provide training to caregivers.
  • Maintain accurate and current records following client/caregiver visits/calls. 
  • Collaborate with Schedulers to ensure requests for additional care are met.
  • Introduce caregivers to clients at the start of care.
  • Provide start of care coverage for clients.
  • Coach caregivers for violating company policies.
  • Participate in staff meetings and events.
 Requirements:

  • MUST have RN or LPN license in the State of Massachusetts.
  • MUST have a valid driver's license, reliable transportation and proof of insurance.
  • Background check is required.
  • Must provide professional references.
  • Must be able to speak, understand and write English.

Qualifications:

  • Bachelor’s degree in nursing or related field preferred.
  • Proven experience as a caregiver or caregiver lead, preferably in the healthcare or home care industry.
  • Strong understanding of home care practices/techniques and services.
  • Excellent communication and interpersonal skills.
  • Flexibility to re-prioritize in a constantly changing environment.
  • Familiarity with home care software to complete documentation.
  • Comfortable with learning various computer programs and ability to work with Microsoft Office.
