Position Overview:
We are currently seeking a highly skilled and experienced Licensed Practical Nurse (LPN) with a background in home health to join our dedicated team. The LPN will be responsible for providing direct patient care in accordance with established care plans, assessing patient needs, collaborating with interdisciplinary healthcare teams, and ensuring the delivery of high-quality care in the home setting.
Responsibilities:
- Conduct comprehensive assessments of patients' health status and develop individualized care plans in collaboration with the interdisciplinary team.
- Administer medications, perform treatments, and provide skilled nursing care to patients in their homes.
- Monitor patients' conditions, assess changes in health status, and communicate effectively with physicians and other healthcare professionals.
- Educate patients and their families about health management, medication administration, and disease prevention.
- Provide emotional support and encouragement to patients and their families, fostering a positive and supportive care environment.
- Document all patient care activities accurately and timely in accordance with company policies and regulatory requirements.
- Coordinate care with other healthcare providers and community resources to ensure continuity of care and optimal patient outcomes.
- Adhere to all applicable laws, regulations, and standards of nursing practice.
Qualifications:
- Current and valid LPN license in the state of Virginia.
- Minimum of 2 years of experience as an LPN in home health or a related setting.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in electronic medical records (EMR) systems.
- Reliable transportation and a valid driver's license.
- BLS certification.
- Ability to pass background checks and drug screenings.
