Job Overview:
We are seeking a dedicated and experienced Registered Nurse (RN) to join our Home Health Agency. The ideal candidate will play a key role in initiating new cases, utilizing OASIS (Outcome and Assessment Information Set), and demonstrating a comprehensive understanding of Medicare Home Health regulations. The RN will be responsible for delivering high-quality, patient-centered care in the comfort of patients' homes, ensuring compliance with all relevant healthcare standards.


Responsibilities:

  1. Case Initiation:
    • Start and manage new cases by conducting thorough assessments of patients' health status and developing individualized care plans.
    • Collaborate with healthcare professionals to ensure a seamless transition of care from hospital to home.
  2. OASIS Utilization:
    • Proficiently use OASIS to document patient assessments, outcomes, and interventions accurately.
    • Ensure compliance with OASIS documentation standards and maintain up-to-date patient records.
  3. Medicare Home Health Compliance:
    • Stay current on Medicare Home Health regulations and guidelines.
    • Implement and adhere to Medicare requirements for documentation, billing, and quality assurance.
  4. Patient-Centered Care:
    • Provide compassionate and evidence-based nursing care to patients in their homes.
    • Educate patients and their families on healthcare procedures, medications, and overall wellness.
  5. Collaboration and Communication:
    • Work closely with interdisciplinary teams, including physicians, therapists, and support staff.
    • Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with care plans.

Qualifications:

  • Active and unrestricted Registered Nurse (RN) license in the state of Virginia.
  • Minimum of 2 years of nursing experience, with a focus on home health.
  • Proficient in OASIS documentation.
  • Strong understanding of Medicare Home Health regulations and compliance.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a collaborative team.
