Responsibilities:
- Conduct care consultations with prospective clients alongside RN staff, addressing their needs, answering questions, and building relationships with clients and families.
- Coordinate and oversee care services, including caregiver selection, scheduling, care plan reviews, caregiver orientation, and supervision.
- Provide ongoing care management, ensuring service quality, client and caregiver satisfaction, and handling scheduling changes and updates.
- Monitor and update care plans based on client needs, communicate changes, conduct home visits, and collaborate with RN staff for reassessments.
- Manage administrative duties, including documentation, policy communication, caregiver training coordination, and participation in company meetings.
- Maintain office hours, provide after-hours/on-call support as needed, and represent the company professionally in the community.
Job Requirements:
- Healthcare experience required. Experience in patient care coordination preferred; senior (geriatric) care preferred. LPN or therapist (PT/OT) license strongly considered.
- Knowledge of medical conditions and terminology, particularly for older adults
- Available for work some weekends and evenings
- Proficient with using software, mobile technology, data entry, and correspondence.
- Ability to interface effectively and professionally with client stakeholders including family, healthcare providers, attorneys, etc.
- Ability to function in a small team environment as well as autonomously
- Excellent written and verbal communication skills
- Travel in OKC metro area required. Mileage reimbursement is provided for job-related travel.
- Must be comfortable working in homes with pets
- Possess a valid driver's license and have use of an insured automobile
- Clean criminal background
- Minimum of 3 professional references

Compensation: $45,000.00 - $48,000.00 per year
