Job Summary
The Social Worker will be responsible for providing comprehensive social work services to clients within our homecare agency. This role involves assisting clients with county assistance discharges and enrollments, navigating the Pennsylvania enrollment broker Maximus, conducting promise eligibility assessments, performing wellness checks, and managing the intake process for new clients. The ideal candidate will be compassionate, highly organized, and possess a strong understanding of community resources and social service programs in Pennsylvania.
Responsibilities
- Facilitate and coordinate county assistance discharges for clients transitioning from various healthcare settings to homecare.
- Assist clients with the enrollment process for various county assistance programs, ensuring all required documentation is submitted accurately and timely.
- Work closely with the Pennsylvania enrollment broker Maximus to ensure seamless enrollment and eligibility for clients under Promise.
- Conduct thorough promise eligibility assessments for prospective clients to determine their qualification for homecare services.
- Perform regular wellness checks for existing clients to assess their overall well-being, identify potential risks, and connect them with appropriate support services.
- Manage the intake process for new clients, including conducting initial assessments, gathering necessary information, and developing individualized care plans in collaboration with the healthcare team.
- Provide crisis intervention and emotional support to clients and their families as needed.
- Act as a liaison between clients, families, healthcare providers, and community resources.
- Maintain accurate and up-to-date client records in accordance with agency policies and regulatory requirements.
- Participate in interdisciplinary team meetings to discuss client care, progress, and challenges.
- Stay informed about changes in social service policies, regulations, and available community resources.
Qualifications:
- Associate or bachelor's degree in social work from an accredited program.
- Minimum of 2 years of experience in a healthcare or homecare setting.
- Demonstrated experience with county assistance discharges and enrollments.
- Proficiency in navigating the Pennsylvania enrollment broker Maximus and understanding Promise eligibility requirements.
- Proven experience conducting wellness checks and managing client intake processes.
- Strong knowledge of local and state social service programs and resources.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a multidisciplinary team.
- Compassionate and empathetic approach to client care.
- Must Have PA Enrollment Brooker Experience. (Maximus).
- Familiarity with standard office equipment such as printers and fax machines
- Excellent computer skills and knowledge of Microsoft Word, Outlook, and PowerPoint
- Highly organized with excellent time management skills and the ability to prioritize projects
- Must have a valid PA/DL
