About the Role:
Join Tradition Home Health Care as a Home Health Aide (HHA-PCA) and make a meaningful impact in the lives of our clients in Brooklyn, NY. We are looking for compassionate and dedicated individuals to provide essential support and care to those in need, ensuring their comfort and well-being.
Responsibilities:
- Assist clients with daily living activities, including bathing, dressing, and grooming.
- Administer medications and monitor health status as directed by healthcare professionals.
- Provide companionship and emotional support to enhance client quality of life.
- Help with light housekeeping and meal preparation to maintain a safe environment.
- Document client progress and report any changes to the supervising nurse.
- Support mobility and physical activities as prescribed in care plans.
- Adhere to all safety and health regulations to ensure client safety.
- Communicate effectively with clients, families, and healthcare team members.
Requirements:
- Valid Home Health Aide (HHA) or Personal Care Aide (PCA) certification.
- Previous experience in home health care or a similar role is preferred.
- Strong interpersonal skills and a compassionate demeanor.
- Ability to follow care plans and instructions accurately.
- Reliable transportation and flexibility in scheduling.
- Basic understanding of medical terminology and health care practices.
- Ability to lift and assist clients as needed.
- CPR and First Aid certification is a plus.

Compensation: $19.50 - $22.00 per hour
