Position Overview
Virginia Home Care Services is seeking a PRN Physical Therapist to join our dedicated team of professionals. This flexible, per-diem role offers the opportunity to provide meaningful care and make a direct impact on patients' lives through in-home physical therapy services.

Key Responsibilities

  • Perform comprehensive assessments to evaluate patients' physical condition and functional abilities.

  • Develop and implement individualized treatment plans based on clinical findings and patient goals.

  • Utilize appropriate therapeutic exercises, techniques, and modalities to enhance mobility, strength, and overall function.

  • Educate patients and caregivers on therapeutic activities, proper techniques, and strategies to support independence and prevent injury.

  • Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.

  • Collaborate effectively with interdisciplinary team members—including physicians, nurses, and occupational therapists—to ensure coordinated and high-quality care.

  • Maintain accurate, timely documentation in accordance with regulatory standards and company policies.

Qualifications

  • Master’s, or Doctorate degree in Physical Therapy from an accredited program.

  • Minimum of 2 years of professional experience as a licensed Physical Therapist.

  • Active Physical Therapy license in the state of Virginia.

  • Current CPR certification.

  • Strong clinical skills with a solid foundation in evidence-based physical therapy practices.

  • Excellent communication, interpersonal, and organizational skills.

  • Demonstrated ability to work independently and manage a varied caseload.

  • Compassionate, patient-centered approach to care.

  • Prior experience in home health or community-based settings preferred.

  • Experience with Home Care Home Base (HCHB) is a plus.

Compensation: $90.00 - $150.00 per hour
