Position Overview
Virginia Home Care Services is seeking a Full Time Physical Therapist to join our dedicated team of professionals. This flexible, per-diem role offers the opportunity to provide meaningful care and make a direct impact on patients' lives through in-home physical therapy services.

Key Responsibilities
- Perform comprehensive assessments to evaluate patients' physical condition and functional abilities.
- Develop and implement individualized treatment plans based on clinical findings and patient goals.
- Utilize appropriate therapeutic exercises, techniques, and modalities to enhance mobility, strength, and overall function.
- Educate patients and caregivers on therapeutic activities, proper techniques, and strategies to support independence and prevent injury.
- Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
- Collaborate effectively with interdisciplinary team members—including physicians, nurses, and occupational therapists—to ensure coordinated and high-quality care.
- Maintain accurate, timely documentation in accordance with regulatory standards and company policies.
Qualifications
- Master’s, or Doctorate degree in Physical Therapy from an accredited program.
- Minimum of 2 years of professional experience as a licensed Physical Therapist.
- Active Physical Therapy license in the state of Virginia.
- Current CPR certification.
- Strong clinical skills with a solid foundation in evidence-based physical therapy practices.
- Excellent communication, interpersonal, and organizational skills.
- Demonstrated ability to work independently and manage a varied caseload.
- Compassionate, patient-centered approach to care.
- Prior experience in home health or community-based settings preferred.
- Experience with Home Care Home Base (HCHB) is a plus.

This is a remote position.
Compensation: $110,000.00 - $120,000.00 per year
