The Patient Care Coordinator serves as the primary point of contact for clients and caregivers. This role ensures that client needs are met, care plans are followed, and caregivers are supported in delivering quality services.

Key Responsibilities:

·        Communicate with clients and families to assess needs and provide updates.

·        Coordinate caregiver schedules to ensure consistent and reliable care.

·        Support caregivers by addressing concerns, providing resources, and helping resolve challenges.

·        Monitor client care plans and adjust as needed.

·        Maintain accurate documentation of client care, caregiver notes, and service updates.

·        Ensure compliance with company policies, state regulations, and quality standards.

·        Act as a liaison between clients, caregivers, and the office team.

Skills & Qualifications:

·        Strong communication and organizational skills.

·        Compassionate, client-focused approach.

·        Ability to multitask and handle scheduling demands.

·        Experience in healthcare, caregiving, or case management preferred.

Working Conditions:

  • Will involve standing, walking, and lifting.
  • Full-time position will require some evening or weekend hours depending on client needs.
Qualifications:

  • Education: High School Diploma or GED required. 
  • Experience:
     
     
    • Minimum 1–2 years of experience in a healthcare setting, preferably in a client-facing role.
