Position Overview:
We are seeking a skilled and experienced Home Health Nurse Coordinator to oversee and support our nursing staff while ensuring the delivery of excellent patient care. This role combines leadership, hands-on clinical training, and coordination across multiple home health assignments. Ideal candidates are flexible, organized, and ready to drive to patient homes as needed.

Service Areas:
Willingness to travel to patient homes throughout assigned service regions (e.g., Fairfax Co, Prince William Co, Loudoun Co, and Stafford Co).

What You’ll Do:

  • Provide hands-on clinical training, guidance, and support to nursing staff

  •  Coordinate with the Scheduling Coordinator on open shift coverage and urgent start-of-care assignments. 

  • Monitor patient care and ensure adherence to care plans and regulatory standards

  • Conduct in-home assessments and occasional direct patient care as needed

  • Serve as a point of contact between patients, families, and nursing staff

  • Assist with office and administrative tasks, including documentation, and reporting

  • Assist with onboarding, mentoring, and professional development of nursing staff

  • Ensure documentation, compliance, and quality assurance standards are met

What We’re Looking For:

  • Active CNA license in Virginia

  • Minimum 3-5 years of home health or caregiving experience

  • Experience in hands-on clinical training and staff mentorship

  • Strong communication, problem-solving, and leadership skills

  • Ability to drive to patient homes; reliable transportation required

  • Flexibility with scheduling to accommodate patient and staff needs

  • Patience, empathy, and dedication to quality patient care
