About the Role:

Wrapped N Love Home Care LLC is seeking a dedicated Intake Coordinator to join our compassionate team in Media, PA. In this vital role, you will be the first point of contact for our clients, ensuring they receive the highest quality of care and support. Responsibilities:

Conduct initial assessments and intake interviews with potential clients.
Gather and document client information accurately and efficiently.
Coordinate with healthcare professionals to develop personalized care plans.
Maintain detailed records and update client information in our database.
Provide exceptional customer service and support to clients and families.
Assist in scheduling appointments and follow-up visits as needed.
Communicate effectively with team members to ensure seamless care delivery.
Stay informed about industry trends and compliance regulations.

Requirements:

Proven experience in healthcare or home care settings preferred.
Strong communication and interpersonal skills.
Detail-oriented with excellent organizational abilities.
Ability to work independently and as part of a team.
Proficient in using electronic health record (EHR) systems.
High school diploma required; relevant certifications a plus.
Compassionate demeanor and a passion for helping others.
Must be able to pass background checks and drug screenings.
