Position Overview
We are seeking a Licensed Practical Nurse (LPN) with strong clinical experience in home health care to join our dedicated team. The ideal candidate will provide skilled nursing care in accordance with established care plans, collaborate with interdisciplinary healthcare teams, and ensure the consistent delivery of safe, effective, and compassionate care in the home setting.
We are seeking a Licensed Practical Nurse (LPN) with strong clinical experience in home health care to join our dedicated team. The ideal candidate will provide skilled nursing care in accordance with established care plans, collaborate with interdisciplinary healthcare teams, and ensure the consistent delivery of safe, effective, and compassionate care in the home setting.
Key Responsibilities
- Conduct comprehensive assessments of patients’ health status and assist in developing individualized care plans.
- Administer medications, perform skilled nursing treatments, and provide direct patient care in clients’ homes.
- Monitor and evaluate patients’ conditions, promptly identifying and reporting any changes in health status.
- Collaborate effectively with physicians, therapists, and other healthcare professionals to coordinate care.
- Educate patients and family members on disease prevention, medication administration, and overall health management.
- Provide emotional support and foster a caring, positive environment for patients and families.
- Accurately document all care provided in accordance with company policies, regulatory standards, and EMR requirements.
- Maintain compliance with all applicable federal, state, and professional nursing regulations and standards of care.
Qualifications
- Current, active LPN license in the State of Virginia.
- Minimum of 2 years of experience in home health or a related clinical setting.
- Strong clinical assessment, documentation, and critical-thinking skills.
- Excellent communication, organizational, and interpersonal abilities.
- Proficiency with Electronic Medical Records (EMR) systems.

BLS certification (current).
- Reliable transportation and a valid driver’s license.
- Must pass background checks
Compensation: $34.00 - $39.00 per hour
