How You’ll Make an Impact
Assess & Plan
Assess & Plan
- Complete in-home RN assessments
- Develop individualized, person-centered Care Plans in collaboration with clients and families
Coach & Supervise
- Orient caregivers to Care Plans
- Perform supervisory and competency visits
- Support caregivers with clinical guidance and best practices
Monitor & Document
- Monitor vitals, condition changes, and client status
- Document accurately in our HIPAA-compliant EMR
- Escalate concerns and changes in condition promptly
Collaborate & Advocate
- Partner closely with Client Care Managers, Care Coordinators, and Scheduling
- Ensure seamless, client-centered care transitions
Quality & Compliance
- Maintain audit-ready care plans and supervisory documentation
- Ensure compliance with state home-care regulations and agency standards
What You’ll Bring
- Active Registered Nurse (RN) license in Maryland and/or Washington, DC
- BSN or related healthcare degree — or 2+ years of home care / geriatric nursing experience
- Strong clinical assessment and critical-thinking skills
- CPR/First Aid certification
- Valid driver’s license and reliable transportation
- Professional, compassionate communication with clients, families, and field staff
