About the Role
We are hiring a Care Manager to own the client experience from start of care through ongoing service.
This role is responsible for ensuring our clients receive consistent, high-quality care and for owning and resolving escalations end-to-end when issues arise.
You will be the primary point of accountability for client satisfaction, working closely with caregivers and the office team to ensure everything runs smoothly.
This is a hands-on role for someone who:

- follows through until problems are fully resolved
Key Responsibilities
- Own the client experience from onboarding through ongoing care
- Conduct new client assessments and develop care plans
- Own all client escalations from identification through resolution
- Take accountability for client satisfaction, retention, and service quality
- Proactively identify and address at-risk clients before issues escalate
- Serve as the primary point of contact for clients and families
- Partner closely with the scheduler to ensure strong caregiver-client matches and full coverage
- Support and coach caregivers in the field to ensure quality care delivery
- Lead communication with clients and families during changes, challenges, or transitions
- Document all client interactions and updates in WellSky
What Success Looks Like
- Clients feel supported, heard, and confident in their care
- Client issues are owned, resolved, and followed through without being passed off
- At-risk situations are identified early and addressed proactively
- Caregivers are aligned and set up for success in each case
- New clients experience a smooth and confident start
Qualifications
- 2+ years of experience in home care, healthcare, or a service-based role
- Strong communication and relationship-building skills
- Ability to manage multiple clients and priorities at once
- Demonstrated ownership and follow-through in previous roles
- Comfortable working in both office and field settings
- Calm, professional approach in high-pressure situations
- Experience with care coordination, case management, or client services is a plus
- Familiarity with WellSky or similar systems is a plus

Compensation: $55,000.00 per year
