Position Summary
The Care Coordinator is a field-based clinical support role responsible for ensuring quality, compliance, and continuity of care across assigned client cases. This position serves as the “eyes and ears” in the home, supporting caregivers, reinforcing care plans, conducting routine client check-ins, and coordinating communication between the field and the office team.
The Care Coordinator works under the direction of the RN Supervisor and Care Manager and does not replace clinical supervision. Instead, this role enhances customer service, caregiver coaching, CareLink compliance, and operational flow.
The primary function of this role is to visit clients regularly, ensure care plans are being followed, support aides in the field, and help structure reporting and workflow as the census grows.
Requirements:
- 1+ year of experience as a Caregiver, Home Health Aide, CNA, or similar role—supervisory duties a plus
- First Aid & CPR certification (or willingness to obtain)
- Proven ability to lead, educate, and inspire caregiving teams
- Strong interpersonal, communication, and problem‑solving skills
- Ability to lift / transfer clients and adhere to all health and safety guidelines
- Reliable transportation, valid driver’s license, and comfort using mobile apps / EMR systems
- Compassionate, respectful, and ethical—professional in every scenario
