JOB DESCRIPTION
Happier At Home Care is a non-medical home care agency located in Bucks County. We are looking for a Full and part time LPN's or MA's with experience in the home care field. Our clients are located throughout Bucks, and Montgomery County.( You will have an assigned area)
Care Coordinator responsibilities will be to ensure that both clients, families and caregivers are assisting the client with the highest quality of care available. The Care Coordinator role has been developed to ensure that all the clients’ needs are being met through our services and that they obtain outside services as needed in the community. The dedicated Care Coordinator will provide the following services.
Responsibilities:
Assessments of new clients
Facilitate client- caregiver meet and greets
Document visits and changes in WellSky
Adress any health issues or concerns
Manage care plan and medication lists
Avalible to cover call out and no show shifts
Train caregiver on tasks pertaining to each individual client
Client, caregiver and family reconsiliation and descalization
Meeting with clients to go over care plan and any changes in care
Random and Announced visits, minimum once a week
Hospitalization visits and Post Hospitalization visits
Communication with the Family
Document Visits in client electronic chart for Happier At Home Care

This position is in the office and mostly on the road, traveling to clients.(Mileage provided)

- Pass background check
- Excel in a high speed work enviornment and multitask
- Ablity to work in a team
- Adiquite technological skills
- Driver's license
- Knowledge of medical terminology
- Strong interpersonal skills with the ability to empathize with patients from diverse backgrounds.
- Excellent organizational skills and attention to detail to manage multiple cases effectively.
