About the Role:
Tradition Home Health Care in Brooklyn, NY is looking for a dedicated Homecare Intake Coordinator to manage the intake process for the Michigan Home Help Program. This is an exciting opportunity to play a vital role in connecting clients with the home-based care services they need, making a real difference in people's lives every day.
Responsibilities:
- Coordinate and process new client intake for the Michigan Home Help Program
- Collect and verify client eligibility, documentation, and authorization requirements
- Serve as the primary point of contact for clients, families, and referral sources during the intake process
- Communicate with Medicaid/state program representatives to facilitate approvals and authorizations
- Maintain accurate and up-to-date client records in the intake management system
- Collaborate with care coordinators, schedulers, and clinical staff to ensure seamless onboarding
- Follow up on pending cases and resolve intake-related issues in a timely manner
Requirements:
- Prior experience in homecare, home health, or healthcare intake coordination preferred
- Familiarity with the Michigan Home Help Program or Medicaid home care programs is a strong plus
- Strong organizational skills with the ability to manage multiple cases simultaneously
- Excellent verbal and written communication skills
- Proficiency in electronic health records (EHR) or intake management software
- Detail-oriented with a commitment to accuracy in documentation and compliance
- Compassionate, patient-focused attitude with strong customer service skills

Compensation: $27.00 - $35.00 per hour
