What We're Looking For
Required:
- Current Florida Physical Therapist license

Medicare-certified Home Health experience
- Experience completing home health evaluations, reassessments and discharges
- Strong clinical documentation and care coordination skills
- Reliable transportation and ability to travel within the assigned territory
- Commitment to timely, patient-centered care
The Role
As a Per Diem Home Health Physical Therapist, you will evaluate patients in their homes, develop individualized plans of care, provide skilled physical therapy interventions, assess progress and coordinate with physicians, nursing and other members of the interdisciplinary care team.
Our goal is simple: deliver excellent clinical care while helping patients remain safe, mobile and independent at home.
