About the Role
Tradition Homecare is looking for a dedicated and experienced Private Duty Homecare Coordinator to join our growing team in Brooklyn, NY.
The ideal candidate will have 3–5 years of hands-on experience coordinating both Private Duty Nursing (PDN) and Home Health Aide (HHA) services. In this role, you will serve as a vital link between clients, families, nurses, home health aides, and our internal team to ensure seamless, reliable, and high-quality care in the home.

Responsibilities
- Coordinate and schedule Private Duty Nurses (RNs/LPNs) and Home Health Aides (HHAs) for client assignments throughout the NYC area
- Manage staffing and coverage for both Private Duty Nursing and HHA cases
- Serve as a primary point of contact for clients, families, nurses, HHAs, and field staff
- Address call-outs, scheduling changes, coverage needs, and other urgent staffing issues
- Assist with client intake, case setup, and coordination of individualized care needs
- Recruit, onboard, and orient qualified nurses and home health aides
- Monitor caregiver and nursing staff attendance, compliance, documentation, and performance
- Maintain accurate and up-to-date client and employee records in accordance with agency requirements and applicable New York State regulations
- Collaborate closely with clinical and administrative teams to maintain continuity and quality of care
- Participate in on-call responsibilities and resolve urgent scheduling or staffing needs as required
Required Experience & Qualifications
- 3–5 years of home care coordination or staffing experience required
- Direct experience coordinating Private Duty Nursing (PDN) cases required
- Direct experience coordinating Home Health Aide (HHA) cases required
- Experience staffing and scheduling RNs, LPNs, and HHAs
- Familiarity with New York State home care requirements and compliance standards preferred
- Strong organizational and multitasking skills with the ability to manage a caseload
- Excellent communication and interpersonal skills when working with clients, families, nurses, aides, and internal staff
- Experience with home care scheduling software, EMR systems preferred
- Bilingual candidates are strongly encouraged to apply

Compensation: $60,000.00 - $90,000.00 per year
