WHAT YOU'LL DO
- Evaluate and treat patients at home. Assess strength, mobility, gait, and range of motion, and evaluate the home environment for safety risks and rehab potential. Deliver treatment under a physician's plan of care.
- Empower patients and caregivers. Teach patients and families how to use and care for therapeutic equipment, and help them understand the goals of the therapy program.
- Guide care. Set treatment priorities, track progress toward goals, and keep physicians informed of any changes in a patient's condition.
- Coordinate care. Serve as case manager when assigned, help plan discharges, and connect patients with other home health services when needed.
- Complete OASIS assessments as assigned.
- Collaborate and mentor. Join case conferences and peer consultations, supervise PT assistants, oversee home health aide care when therapy is the only skilled service, and lead in-service education for colleagues.
WHAT WE'RE LOOKING FOR
Required
- A degree in physical therapy (bachelor's, master's, or DPT) from an accredited program
- An active California Physical Therapy Board license
- Current CPR certification
- A valid driver's license and an insured, reliable vehicle
- Strong communication and organizational skills
Preferred
- One or more years of PT experience
- Home health or community-based experience
WHAT TO EXPECT
This is a field-based role, so you'll be driving to patients' home within our service area. As with any Home Healthcare job, you may be exposed to infections and contagious diseases, and no two situations are the same. You'll be ready for the unexpected, including the occasional emergency.

Compensation: $75.00 - $95.00 per hour
