Tradition Homecare - NY is looking for a compassionate and skilled Private Duty Homecare Nurse to join our dedicated team in Brooklyn, NY. Responsibilities:
- Provide direct, private duty nursing care to patients in their home environment
- Assess, monitor, and document patient health status and progress
- Administer medications, treatments, and medical procedures as prescribed
- Develop and implement individualized patient care plans
- Communicate effectively with patients, families, and healthcare team members
- Educate patients and caregivers on health management and best practices
- Maintain accurate and timely clinical documentation and records
Requirements:
- Active RN license in the state of New York
- Prior home care, private duty, or similar nursing experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently with minimal supervision
- Reliable transportation and ability to travel to patient homes in Brooklyn, NY
- CPR/BLS certification required

Compensation: $80.00 - $100.00 per hour
