Always Responsive Home Care is looking for an outgoing, energetic Community Care Liaison to join our growing team!
We’re especially interested in people who have worked in skilled nursing facilities, assisted living communities, hospitals, or rehabilitation centers and understand how much strong relationships with healthcare professionals matter.
Do you love working with people, building relationships, and making a difference for seniors and their families? Whether your background is in activities coordination, social work, admissions, or another healthcare role, imagine taking the skills you already have beyond the walls of one facility. This is an on-the-road role for someone who enjoys connecting with people, bringing communities together, and helping others. Instead of spending your day in one building, you’ll visit healthcare facilities, build relationships with healthcare professionals, organize engaging events, and connect families with the care they need. If you thrive on personal interaction, enjoy variety in your workday, and love making meaningful connections, this could be your perfect next step.
What You’ll Be Doing
- Build and maintain relationships with social workers, discharge planners, case managers, activities directors, and other healthcare professionals
- Visit hospitals, skilled nursing facilities, rehabilitation centers, assisted living communities, and other referral partners daily
- Develop new referral partnerships while strengthening existing ones
- Plan, organize, and host networking events, educational presentations, and community outreach activities
- Represent Always Responsive Home Care at local networking events and professional gatherings
- Educate healthcare professionals about our services and how we can support their patients, residents, and families
- Manage your own daily schedule and territory, keeping a consistent presence in the community
- Track and report key results, including outreach activities, referrals, and relationship development
- Work toward clear business development and referral goals
What We’re Looking For
- Background in social work, activities programming, admissions, discharge planning, senior living, or healthcare preferred
- Experience in a hospital, skilled nursing facility, rehabilitation center, or assisted living community strongly preferred
- Outgoing, personable, and confident, with excellent communication skills
- A natural relationship builder who enjoys networking and meeting new people
- Self-motivated, organized, and comfortable working independently in the field
- Goal-oriented and comfortable being accountable for results
- Valid driver’s license, reliable transportation, and willingness to travel throughout the Lehigh Valley
Previous sales or marketing experience is not required. We’re looking for the right personality, healthcare experience, and drive to succeed.
Compensation
- Base salary of $50,000 - $60,000, based on experience
- A bonus for every new client family you help us serve, plus quarterly goal bonuses
- Mileage reimbursement and company cell phone
