Caring Hands Home Health, Inc. is seeking a dedicated and compassionate PRN RN Case Manager to support our clients in High Point, Archdale, and Thomasville, NC.

Key Responsibilities:
- Conduct in-home assessments and develop care plans
- Educate patients and caregivers on treatment plans and health management
- Perform supervisory visits every 90 days or 60 days for CAP cases
- Document patient interactions in a mobile EMR system
- Travel within High Point, Archdale, Thomasville, and surrounding areas
- Collaborate with field staff and office team to ensure high-quality care
Qualifications:
- Active RN license in NC or a compact/multi-state license
- Graduate of an accredited nursing program
- Minimum of 2 years of nursing experience (home health preferred)
- Strong clinical judgment and communication skills
- Reliable transportation and mobile device for documentation
- Ability to work independently and travel for client visits

Compensation:
Competitive hourly rate based on experience. Weekly pay. This is a PRN (as needed) position.
