Care Right There Home Care is seeking a highly skilled, compassionate Hourly Activities & Dementia Care Specialist who will work across multiple client homes as a Client Support Supervisor. This role is ideal for an experienced CHHA who is passionate about dementia care, believes in rehabilitation, confident in communication, and dedicated to advocating for clients and families.

Role Summary

This role is a hybrid between a Caregiver, Field Support Supervisor, and Client Advocate. You will rotate between cases to:

  • Monitor clients
  • Check on caregivers
  • Identify gaps in care plan to care 
  • Train and counsel staff
  • Introduce and orient staff
  • Evaluate care quality
  • Support dementia-related needs
  • Ensure technology tools located in home are both set up and being used properly
  • Reinforce training and best practices
  • Report to nurse and care team regarding findings/ weekly reports 
  • Responsible for metrics and key performance indicators ( we will train for this)

Your mission is to help every client receive the right care at the right time by being proactive, supportive, and engaged.


Key Responsibilities

Client Support & Supervision
  • Visit assigned clients regularly to assess well-being and monitor changes
  • Ensure caregivers are following care plans, routines, and essential safety standards
  • Assist clients with holistic and dementia-related needs using best-practice techniques
  • Provide companionship, reassurance, and cognitive engagement activities
  • Document observations clearly and promptly in required systems. 
  • Occasionally introduce, orient and run supplies to and from client 
  • Technology set up and check up as well as collection,

Technology Monitoring & Compliance
  • Use AI tools to review  insights, detect patterns, and identify potential concerns
  • Review RYTCare360 tasks and activities to ensure they are being completed
  • Support caregivers in using technology tools correctly
  • Report concerns or anomalies immediately to office/nursing team

Training & Coaching
  • Mentor caregivers on activity and dementia communication, redirection techniques, and safe care
  • Reinforce agency training standards and proper documentation practices
  • Demonstrate how to complete RYTCare360 Activities and care tasks correctly
  • Provide one-on-one coaching when caregivers need hands-on guidance

Care Plan Oversight
  • Ensure each caregiver is following the client’s care plan accurately
  • Monitor progress on client-specific goals and report improvements or decline
  • Provide input to RN/Lead Staff when updates are needed
  • Encourage therapeutic activities such as cognitive exercises, sensory tasks, music therapy, and physical activity (as approved by RN)

Client Advocacy
  • Serve as an additional set of eyes and ears for the office
  • Build relationships with families and communicate concerns respectfully
  • Ensure clients feel valued, supported, and treated with dignity
  • Identify unmet needs or safety issues proactively


Requirements
  • Active NJ CHHA License (required)
  • Strong experience working with Dementia/Alzheimer’s clients
  • Ability to travel between client homes (reliable transportation required)
  • Strong communication, teaching, and problem-solving skills
  • Comfort using technology tools such as:
    • Sensi AI (not required but nice to have)
    • AxisCare (not required but good to have)
    • Mobile apps, tablets, basic tech
  • Professional, compassionate demeanor with strong client engagement skills
  • Ability to document thoroughly and accurately
