Personal Care Nurse Case Manager – Client Relations & Retention

Virginia Home Care Services is seeking a compassionate and relationship-driven Personal Care Case Manager to support client satisfaction, communication, and long-term retention. This role focuses on building strong relationships with clients and families while ensuring high-quality service delivery.

Position Overview

The Personal Care Case Manager serves as a primary point of contact for clients and their families. This role emphasizes client engagement, service satisfaction, issue resolution, and ongoing relationship management to promote continuity of care and long-term client retention.

Key Responsibilities

  • Build and maintain strong relationships with clients and family members

  • Serve as a trusted point of contact for client concerns, questions, and feedback

  • Conduct regular client check-ins to assess satisfaction and care quality

  • Address and resolve service concerns in a timely, professional, and empathetic manner

  • Support client retention through proactive communication and follow-up

  • Coordinate with internal teams to ensure client needs and expectations are met

  • Document client interactions and service updates accurately

  • Participate in care plan discussions and service reviews as needed

  • Represent the company professionally in all client-facing interactions

Qualifications

  • Experience in home care, healthcare, client relations, or case management preferred

  • Strong customer service and interpersonal communication skills

  • Ability to de-escalate concerns and maintain positive client relationships

  • Detail-oriented with strong documentation skills

  • Bilingual (English/Spanish) preferred but not required

  • Professional, compassionate, and client-focused mindset

  • LPN required, RN preferred
