Homewatch CareGivers of Yorba Linda is seeking an experienced Client Care Manager who is exceptional at building relationships with seniors, family members, caregivers, and healthcare professionals. This is a highly visible, client-facing leadership position for someone with a big heart, excellent instincts, and the confidence to step in and take charge when a situation becomes difficult.

The right person will make every client feel like our most important client, while still being able to establish boundaries, have honest conversations, and hold people accountable when necessary.

The Role:

The Client Care Manager is the primary relationship owner for our clients and their families. You will spend significant time visiting clients in their homes, understanding their needs, resolving concerns, supporting caregivers, and making sure the care we promised is the care we are delivering.

This is not primarily a desk position. It requires someone who enjoys being in the field, connecting with people, and responding quickly when circumstances change.

Your responsibilities will include:

  • Building strong, trusted relationships with clients and family members
  • Conducting regular in-home client visits, wellness checks, and reassessments
  • Developing and updating individualized plans of care
  • Helping new clients and families feel comfortable and supported from the beginning of service
  • Recognizing changes in a client’s condition, environment, or care needs and responding promptly
  • Conducting home-safety reviews, fall-risk assessments, and cognitive screenings within the scope of our non-medical services
  • Coordinating care conferences with clients, families, caregivers, and members of our office team
  • Addressing client concerns with urgency, empathy, sound judgment, and appropriate follow-through
  • De-escalating difficult situations while protecting the client relationship and maintaining company standards
  • Coaching caregivers in the field and reinforcing expectations connected to each client’s plan of care
  • Partnering closely with scheduling, operations, recruiting, and leadership to resolve problems before they become larger issues
  • Maintaining accurate, timely documentation of client visits, changes, and follow-up actions
  • Bringing ideas, concerns, and solutions to our leadership team as we continue to grow
Who Will Thrive in This Position

You also understand that great client service is more than being friendly. It requires strong follow-through, attention to detail, accurate documentation, and the ability to coordinate many moving pieces at once.

Preferred Experience

We are especially interested in candidates with experience in:

  • Non-medical home care
  • Senior care or aging services
  • Assisted living, memory care, hospice, home health, or another healthcare setting
  • Client care coordination, case management, patient relations, or family support
  • Nursing or another clinical discipline
An LVN, RN, or other healthcare professional could be an excellent fit, although a nursing license is not required. We are a non-medical home care agency, but healthcare knowledge and sound clinical instincts can be valuable when recognizing changing needs and communicating with families.

Spanish-English bilingual skills are a strong plus but are not required.

What We Are Looking For

  • At least 3-5 years of relevant home care, healthcare, senior care, or client-management experience
  • Direct experience working with clients, patients, seniors, and family members
  • A proven ability to build relationships and manage difficult or emotional conversations
  • Strong verbal and written communication skills
  • Excellent organization, documentation, and follow-through
  • The ability to make sound decisions and respond calmly when plans change
  • Confidence working independently in the field while remaining closely connected to the office team
  • Comfort speaking up, contributing ideas, and participating as a member of the leadership team
  • A valid driver’s license, reliable vehicle, and current automobile insurance
  • The ability to travel regularly between client homes throughout our service area
Schedule and On-Call Participation

Home care is a 24-hour business, even though this is primarily a weekday position.

This is a full-time role with a regular Monday-through-Friday schedule. It also includes:

  • Occasional weekend or after-hours involvement when a client situation requires leadership support
  • Participation in a shared emergency on-call rotation with approximately four to five members of the office team
  • Regular travel to client homes throughout the workday
The on-call responsibility is shared. This is not a position in which one person is expected to carry the phone every night or every weekend. However, every member of our leadership team must be willing to step in when our clients, caregivers, or teammates genuinely need support.

Compensation and Benefits

  • Compensation: $70,000-$75,000 annual salary, based on directly related experience
  • Full-time position
