Azalea Homecare is seeking an organized, dependable Care Coordinator to help manage client services, caregiver scheduling, staffing, and day-to-day communication. This role serves as a primary point of contact for clients, families, and caregivers and works closely with operations, nursing, recruiting, onboarding, and billing.

Responsibilities
Client & Family Coordination
- Serve as a primary point of contact for clients, families, and caregivers.
- Maintain regular communication regarding schedules, caregiver changes, concerns, and service needs.
- Complete weekly client check-ins by phone, email, or in person as appropriate.
- Follow client concerns through resolution and escalate significant service, safety, or staffing issues when needed.
- Ensure client preferences and care plan requirements are clearly communicated to caregivers.
- Coordinate client-to-caregiver introductions.
Scheduling & Staffing
- Create and maintain caregiver schedules based on client needs, caregiver availability, location, and agency requirements.
- Coordinate caregiver assignments, call-outs, vacations, schedule changes, and backup coverage.
- Respond quickly to same-day staffing issues and secure appropriate coverage.
- Help maintain a sufficient PRN caregiver pool for call-outs, new clients, and changing service needs.
- Communicate current and upcoming staffing needs to the Recruitment Specialist and Director of Operations.
Documentation & Coordination
- Maintain accurate client information, notes, schedules, and follow-up tasks in WellSky.
- Document important client and caregiver communication.
- Review assigned EVV issues, missing documentation, schedule discrepancies, and other service-related exceptions.
- Assist with new client starts and coordinate with nursing, scheduling, onboarding, billing, and management.
- Help track outstanding client documentation and service-related follow-up items.
- Assist with timesheet and payroll-related information as needed.
Recruitment & Onboarding Support
- Participate as part of the caregiver onboarding team.
- Assist the Recruitment Specialist with staffing needs and caregiver placement.
- Help place newly hired caregivers into available client schedules.
Team Accountability
- Attend daily and weekly team huddles focused on client care, staffing, new clients, outstanding issues, and process improvement.
- Maintain awareness of open shifts, upcoming client starts, unresolved client concerns, and staffing risks.
- Provide clear updates to the Director of Operations regarding outstanding items and next steps.
- Answer calls, emails, and other communications professionally and promptly.
Qualifications
- Previous experience in home care, healthcare scheduling, staffing, client services, or a related role expected.
- Strong communication and customer-service skills.
- Excellent organization, problem-solving, and follow-through.
- Ability to manage multiple clients, caregivers, schedules, and priorities.
- Comfortable using electronic scheduling and documentation systems.
- Experience with WellSky, EVV, Medicaid home care programs, or private-pay home care is a plus.
Compensation: $23.00 - $25.00 per hour
